Most UK fellowships are hospital appointments
In the UK, a plastic surgery fellowship is usually a post created by an NHS trust or health board, often at senior clinical fellow or trust fellow grade. It is a job with a contract, rota and clinical duties. That means three separate things must line up: appointment by the employer, registration with a licence to practise from the GMC, and permission to work in the UK.
Fellowship titles vary. Read the person specification for the grade, the registration it requires, the expected operative role and whether the post includes on-call work.
GMC registration routes
The GMC publishes several routes to full registration with a licence to practise for international medical graduates, including passing the PLAB examinations, holding an acceptable postgraduate qualification, and sponsorship by an approved sponsor. Each route has its own conditions; check the route that matches your qualifications before applying for posts that start soon.
Allow time for verification of your qualifications and for evidence of English language capability. A trust can make an offer conditional on registration, but cannot grant registration itself.
The MTI scheme has closed to new applicants
The Medical Training Initiative, which allowed international doctors up to two years of NHS training with the Academy of Medical Royal Colleges as national sponsor, closed to new applicants at noon on 31 March 2026. NHS Employers states that applications already in progress could be completed until 30 June 2026 and that current participants are supported for the rest of their placements.
Older guides and vacancy notices may still describe MTI as an option. Check whether any post you are considering refers to another route, and confirm the visa sponsorship arrangements with the employer.
Visas for employed posts
Doctors employed by the NHS or by organisations providing medical services to the NHS commonly apply for the Health and Care Worker visa, which needs a job offer and certificate of sponsorship from a Home Office approved employer. Salary thresholds, fees and eligible roles change, so use the live GOV.UK guidance rather than figures quoted in older sources.
JCST interface fellowships are for UK-certified surgeons
The JCST interface fellowships, including hand surgery, cleft lip and palate, and oncoplastic breast surgery, are post-certification posts for surgeons on the UK specialist register, ideally within two years of certification. They are not general entry routes for surgeons trained abroad, although comparable subspecialty experience may be available in trust fellowship posts at units that host them.
Questions to take to the programme
- Which GMC registration route does the trust expect for this post?
- Will the trust sponsor a Health and Care Worker visa for this grade?
- What operative role, supervision and on-call commitment does the post include?
- Is the post tied to a particular start date, and what happens if registration is delayed?
